They should really consider raising the lower bound if fines are meant to act as any kind of deterrent. I get that a first offense warning should be lighter but it should still sting a little. This range is way too wide.

I very much doubt it. That would be the very clearest case of denying a goal scoring opportunity that wasn't a pen.

 

Would be a free kick and red all day long.

The position of the covering  defender may have come into a red card decision .

Probably not, since the ball was headed directly into the empty net. The covering defender is irrelevant.
