To me that looks like the main reason why they can't string more than two passes together. There's no recognized pattern of play. The players with the ball have no idea where team mates are, will be or what runs they are likely to make. It's worst when the defenders have the ball, even in acres of space they are scared to carry the ball forward, they end up just passing it uselessly amongst themselves until it ends up being hoofed forward.
